could also download the form to your device as a PDF,鈥 then open it on your device, fill in and attach it to an e-mail and send to mloff@westernsd.mb.ca.鈥 If you submit the registration forms online, feel free to send a copy or photo of the required documents listed below via e-mail to 尘濒辞蹿蹿蔼飞别蝉迟别谤苍蝉诲.尘产.肠补鈥 or stop in at the school and we can copy it for you. 鈥媁e require the following documents when registering your child at school either online or in person:
- 鈥媋 birth certificate or other legal document with legal name and birthdate
- a copy of your child's permanent resident document or student/visitor visa document if you are new to Canada
- proof of address (utility bill, driver's license)
Any families requesting a school outside of their school catchment area (with the exception of French Immersion) will need to fill out a School of Choice form. These are available on our school website under the title 鈥淩egistration Information" or at the school office. 鈥
鈥嬧媁e will always confirm receipt of your child's registration once received. If you have not received a confirmation email, please contact us.
